Has a cocktail mode ever been implemented in Wah!Cade?

Partially. It's possible to have layouts with rotated labels & lists.

Well I guess I need to ask. if I install a Vertical or cocktail table layout will the controllers correspond to the joystick movement? In other words will the up press of the joystick get the selection to go up on the list?

I suppose that would depend on how you've got your controls laid out. Do you have a two or three sided control panel? (i.e. one set of joysticks for "horizontal" games and another set for "vertical" games). You can have more than one keypress / joystick action defined to wahcade actions.
